KYZIEN PROVIDER AGREEMENT
This Provider Agreement (“Agreement”) is entered into between Kyzien (“Platform”) and the registering professional or business (“Provider”).
1. Independent Contractor Status
Provider is an independent contractor and not an employee, agent, or representative of Kyzien.
Kyzien does not control or supervise the services provided.
2. Provider Responsibilities
Provider agrees to:
Maintain all required licenses, certifications, and insurance
Provide accurate and truthful information
Deliver services in a professional and lawful manner
Comply with all applicable laws and regulations
3. No Medical Oversight
Kyzien does not provide clinical oversight or supervision.
Provider is solely responsible for:
Client care
Treatment decisions
Outcomes
4. Listings & Content
Kyzien may:
Review, edit, or remove listings
Suspend accounts for misleading or false information
Provider may not:
Misrepresent credentials
Post misleading claims or guarantees
5. Payments & Fees
Kyzien may charge platform or service fees
Payment processing may be handled by third parties
Providers are responsible for taxes
6. Insurance Requirement (STRONGLY RECOMMENDED)
Providers are responsible for maintaining appropriate liability insurance.
7. Indemnification
Provider agrees to indemnify and hold Kyzien harmless from any claims arising from:
Services provided
Client interactions
Violations of law
8. Termination
Kyzien may suspend or terminate Provider access at any time for:
Policy violations
Risk to users
Fraud or misconduct
9. Limitation of Liability
Kyzien is not liable for:
Loss of income
Client disputes
Damages arising from Provider services
